- What was the input? The prompt or instruction given to the AI, including any system prompts, reference materials, or data inputs. This must be recorded verbatim, not summarised.

Layer 1: Automated Interaction Logging
Every AI interaction should be logged automatically. If your team uses AI through an API, this is straightforward: API calls are logged with timestamps, inputs, outputs, and model parameters. If your team uses AI through consumer interfaces (ChatGPT web app, for instance), you need either a browser extension that captures interactions or a policy requiring copy-paste documentation into a central system.

Layer 2: Review Documentation
Your content management workflow needs a compliance layer. Most content management systems can be configured to capture this information with minimal customisation. The requirement is building it into the workflow so it happens by default, not by choice.
Layer 3: Retention and Retrieval
Records must be stored in a searchable, immutable format with appropriate retention periods.
